Dear Sir/Madam,

I am a resident of the __________ locality. I have obtained a water connection from the corporation for my house at (Address). I have been using this water connection for over ___ years.

I would like to bring to your attention that the supply of water has been very irregular since the last few months. Generally, the water supply is given every ___ days for ___ hours in the morning and ___ hours in the evening. However, I have been noticing, nowadays, the water supply is given at delayed intervals every ___ days instead of ___ days. The duration of water supply is also reduced to ___ hours in the morning and ___ hours in the evening.
