Affidavit Format for Lost Drivers Licence (Duplicate DL) in India

You may be asked to submit an affidavit while making an application for the issue of a duplicate driving licence for the lost, misplaced, stolen, or destroyed driving licence of yours.

An affidavit is a written statement confirmed by oath or affirmation by a person on his personal capacity. As per the law of the country, an affidavit can be used as evidence in court. Therefore, it is important that you make truthful statements in the affidavit.

If it is proved that you have made any wrong statements knowingly in the affidavit, then it may land you in jail for some time and/or be penalised with fine.

Here is a sample of an affidavit for loss of driving licence and issue of duplicate driving licence.

5. That my driving licence has not been cancelled nor been impounded by any statutory authority for any violation of law.

6. That I have applied for the issue of a duplicate driving licence and have submitted the required fee.

7. That I am qualified to hold the driving licence.

8. That I shall abide by the provisions of any Act & Rules for the motor vehicles made under the law of the State of __________ for the time being in force.

9. That if at any time in the future the original licence comes into my possession, I undertake to deposit the same with the RTO, _______.
